What we collect through this website
When you send a booking request through the form on our website, we collect only what we need to arrange your appointment:
- Your name, phone number and email address
- Whether you are a new or existing patient, and the reason for your visit
- Your preferred date and time
- Your medical aid, if you choose to give it
- Anything you tell us in the notes field
The reason for your visit and anything you write in the notes field may amount to information about your health, which POPIA treats as special personal information. We ask you to share only what helps us prepare for your appointment — please don't send detailed medical history through the website.
Why we use it, and on what basis
We use this information solely to contact you, confirm your appointment and prepare for your visit. We process it because you have asked us to arrange dental care for you, and because processing is necessary for the provision of medical services by a health care professional bound by a duty of confidentiality.
Who else sees it
- Our website and form host. The site is hosted on Netlify, which receives and stores your form submission and passes it on to the practice.
- Email. Booking requests reach the practice by email, so the request passes through our email provider.
- Your medical aid scheme, but only where you are a patient and we submit a claim on your behalf.
- Google, if you choose to load the map on our contact section. The map is not loaded until you click it, and we do not run advertising or analytics trackers on this site.
We do not sell your personal information, and we do not share it for marketing.
How long we keep it
If you do not become a patient, we keep your booking request only as long as we need it to respond to you. Once you are a patient, your clinical records are kept for the period required of health practitioners by the Health Professions Council of South Africa.
How we protect it
The website is served over HTTPS, and access to booking requests and patient records is limited to the practice. Patient files are kept confidential in the ordinary course of dental practice.
Your rights
Under POPIA you may ask us to confirm what personal information we hold about you, to correct it if it is wrong, or to delete information we no longer have a legal reason to keep. You may also object to how we use it, and you may withdraw a booking request at any time. Write to drmcassim@gmail.com or call 011 883 3005 and we will respond as promptly as we can.
If you believe we have not handled your personal information properly, you are entitled to complain to the Information Regulator (South Africa) — inforegulator.org.za. We would appreciate the chance to put it right first.
Children
Where we treat a child, we collect their information from a parent or guardian, with that parent or guardian's consent.
Changes to this notice
We will update this page if the way we handle personal information changes.
